1. About the Service

Carolyn Ford Physiotherapy Services provides assessment, treatment and advice for children requiring           physiotherapy support. Services will be delivered at home, or remotely where appropriate.

2. Appointments

• Appointments must be booked in advance.

3. Fees and Payment

• Fees will be communicated in advance of treatment.
• For initial assessment fees must be paid in advance.
• For follow up appointments payment is due at the time of the appointment unless otherwise agreed.
• Accepted payment methods will be confirmed at booking.
• Invoices must be settled within 7 days unless agreed otherwise.

4. Cancellation Policy

• A minimum of 24 hours’ notice is required to cancel or reschedule an appointment.
• Cancellations made with less than 24 hours’ notice may be charged at the full appointment rate.
• Non-attendance without notice will be charged in full.

5. Medical Information and Consent

• Patients (or parents/guardians) must provide accurate and complete medical information.
• Treatment will only proceed with informed consent.
• For patients under 18, consent must be provided by a parent or legal guardian.

6. Safeguarding and Chaperones

• A parent or guardian must be present for all appointments involving children.
• Safeguarding policies are followed in line with professional standards.

7. Treatment and Outcomes

• Physiotherapy outcomes can vary and cannot be guaranteed.
• Advice and exercises provided form part of treatment and should be followed as agreed.
• Failure to follow advice may affect outcomes.

8. Confidentiality and Data Protection

• All personal information is handled in accordance with the UK General Data Protection Regulation (UK GDPR).
• Records are stored securely and retained in line with professional requirements.
• Information may be shared with other healthcare professionals only with consent, unless required by law.

9. Reports and Medico-Legal Work

• Reports (including medico-legal reports) are subject to separate fees and timescales.
• Draft and final reports remain the property of Carolyn Ford Physiotherapy Services until payment is received in full.

10. Liability

• All reasonable care is taken to provide safe and effective treatment.
• Carolyn Ford Physiotherapy Services is not liable for any injury or loss arising from failure to disclose relevant medical information or failure to follow professional advice.

11. Complaints

• Any concerns should be raised directly in the first instance.
• If unresolved, patients may escalate concerns in line with the complaints procedure and professional regulatory guidance.

12. Professional Registration

• Carolyn Ford is registered with the Health and Care Professions Council (HPCP number PH54647), Chartered Society of Physiotherapists (Membership number 059697) and Association of Paediatric Physiotherapists, and adheres to professional standards and codes of conduct.

13. Insurance

• Appropriate professional indemnity and public liability insurance is in place through the Chartered Society of Physiotherapy and certificate can be provided if requested.

14. Changes to Terms and Conditions

• These Terms and Conditions may be updated from time to time.
• The latest version will be available upon request.

15. Governing Law

• These Terms and Conditions are governed by the laws of Scotland.
